NLCSD will be offering district wide physicals for those students who are unable to get appointments with their primary doctor or are past due on school physical requirements. These physicals can also be used for students participating in school sports. The NL District Medical Director will be performing DISTRICT PHYSICALS on October 24th during school hours.
Per New York State Commissioner’s Regulations 136.3 (c) (1) (iii), students entering grades Pre-K, K, 1, 3, 5, .0 to the district, are required to have a physical exam. If your child is entering these grades, they will require an updated physical for the 2023-2024 School Year.
Families will need to fill out the permission slip below in order to receive a physical.
