Dear WBH Parents:
Per New York State Commissioner’s Regulations 136.3 (c) (1) (iii), students entering grades Pre-K, K, 1st, 3rd, 5th, and NEW entrants to the district, are required to have a physical within 30 days of starting school.
It is recommended that you obtain this with your private health care provider. If your child has had an updated physical, please send it into the health office. If not, please call your child’s doctor as soon as possible to schedule a physical.
NY schools cannot accept the health exam if it is not on the required form or the required health record equivalent.
