COVID-19 Vaccination Clinic
When – Tuesday, May 4th:
1:15-1:45 – New Lebanon High School Students Only (16 and over)
1:45-3:15 – Open to the public
Where – New Lebanon Jr./Sr. High School Cafeteria

New Lebanon CSD is partnering with Columbia County Department of Health to offer the Pfizer COVID-19 vaccination to students over 16 year of age, whose parents give permission. We had several families show interest and had already handed in a permission slip. This permission slip allows for a guardian not to be in attendance while their child is receiving his or her vaccination. The Department of Health informed us that they also need families to make a reservation at the link below. We are asking families who give permission for the DOH to vaccinate their child and want this done during the school day (without a parent in attendance) to choose a slot between 1:15 – 1:30 or 1:30 – 1:45. This will allow for your child to take the bus home at the end of the day. If a parent is not in attendance, we also need families to complete a permission slip. For a permission slip, please contact the Jr./Sr. High School nurse at jblenke@newlebanoncsd.org. If you choose a slot after the 1:45 time slot, you will have to attend the clinic with your child.
The time slots after 1:45 are also open to the general public who are 16 and over (parents, grandparents, friends etc.). Please spread the word.
